Technical Assistance:
Consultation was given on using the model DSS medication authorization forms and on-line training opportunities for providers.

Comments:
An unannounced renewal study ws conducted today from 9:50-10:55. Eight children (21 points) were with two caregivers which is within ratios for supervision. Observed infant care, free play with blocks, puzzles and trucks, bottle feeding and transition to crib for the infant in care. Space is clean and well organized. Reviewed 3 children's records, both caregivers and emergency preparation. Lunch to be served today was cheese and bologna sandwich, veggies and fruit. Training is current for the annual requirement as well as first aid, MEdication Administration Training (MAT) and CPR certification. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-700-A
Description: Based on record review, written authorizations for medication had expired.
Evidence:
Two written authorizations for Child #1 have expired as the date the parent and physician signed the forms are older than 6 months.

Plan of Correction: Give parents new form.

Standard #: 22VAC40-111-100-A
Description: Based on record review, 1 of 3 children's record did not contain documentation of a physical exam within 30 days of attendance.
Evidence:
Child #3 did not have a physical exam record in the file (enrolled 3/2017).

Plan of Correction: Receive updated record.

Standard #: 22VAC40-111-90-C
Description: Based on record review, 1 of 3 children's records was missing an updated immunization record.
Evidence:
Child #3 was missing additional immunization documentation every 6 months. Last immunization record was 4/2018.

Plan of Correction: Receive from paernts updated records.

Standard #: 22VAC40-111-180-A
Description: Based on record review, each caregiver/adult household member did not have a current TB form every two years from the date of the previous screening.
Evidence:
Both caregivers' TB forms expired 9/2019.
