Comments:
An unannounced monitoring inspection was conducted on January 7th from 10:30am through 11am and on January 10th form 10am through 11:30am. There were 9 children in care, on both days, ranging from ages 16 months through four years old. Children present for care on the 7th totaled 20 and 21 on the 10th. There were three caregivers present. On January 7th children were observed watching television and eating lunch. On January 10th children were observed , engaged in teacher and self-directed play activities including story time, music and movement and playing with toys and blocks, eating lunch, restroom routines and transitions. Children and caregiver records were reviewed as well as required postings, emergency drills and emergency items. The provider stated that there is not any medication being administered to children in care. The provider stated that children in care are not transported by the family day home. Violations in the areas of Physical Health of Caregivers and Household Members and Care of Children were identified and appear on the violation notice. These violations were reviewed with the provider at the conclusion of the inspection.

Description: Based on record review and interview the family day home failed to ensure that tuberculosis screening are completed within 30 days.
Evidence:
1 - The documentation for the tuberculosis screening for staff 2 is dated September 12, 2019. The documented date of hire for staff 2 November 14, 2019. Therefore, the screening documentation was outside the allotted 30 days.
2 - Staff 1 confirmed that that the documentation of tuberculosis screening was outside the allotted 30 days.

Standard #: 22VAC40-111-580-C
Description: Based on observation and interview the family day home failed to provide opportunities for vigorous outdoor play daily.
Evidence:
1 - Despite unseasonably warm weather, during the past week, with sunny conditions and temperatures in the mid to upper 50's and 60's, staff 1 and 2 stated that children have not been outside all week.
2 - Children were not observed in outside play during the inspection on January 7 or 10th.
