Comments:
The licensing inspectors conducted an unannounced renewal inspection on 01/10/2020 from 9:35am to 2:45pm. The children were playing upon the inspectors' arrival, and they were later observed playing with toys and eating lunch. Today, there were five children, totaling 18 points, present with two caregivers. The caregivers were observed interacting with the children and cleaning. Interviews were held with the caregivers throughout the inspection. Additionally, when appropriate the inspector interacted with children. Lunch consisted of meatballs, rice, vegetables, pineapples and milk.

The provider does not administer medication.
The provider does not provide transportation.
The provider does not have pets.

Five children?s records, and two caregivers? records were reviewed during the inspection.

The violations from the previous inspections were checked for corrections. There were two repeat violations found during today?s inspection.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not ensure two of five children's records contained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. The record of child #4 (start date: 2/19/19) had documentation of a physical examination dated 6/14/19.

Plan of Correction: I'll make sure it is done before they start.

Standard #: 22VAC40-111-210-A
Description: Based on record review, the provider did not ensure to obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Evidence:
The provider had documentation of 10 annual training hours. Caregivers must obtain 16 hours of annual training.

Plan of Correction: We will be starting training every three months starting this month.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not ensure that areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence:
The inspector observed the following:
1. The provider's back yard had a piece of sharp splintering wood located within the reach of children.
2. The provider's fence had sharp rusted areas on the chain link fence within the reach of children.
3. The provider's deck had three loose boards which presented a tripping hazard.

Plan of Correction: I will fix all of the above issues.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider did not ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.
Evidence:
The inspector observed the following:
1. There were two substances located under an unlocked bathroom cabinet within the reach of children in care. Both substances were labeled with warnings that stated for the consumer to keep out of reach of children.
2. The provider had five deodorizers located throughout the home. Some of the deodorizers were within the reach of children. The deodorizers were labeled with warnings that stated for the consumer to keep out of reach of children.

Plan of Correction: Corrected during the inspection.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure outlet covers were installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
There were three uncovered outlets located on and near the provider's back porch.

Standard #: 22VAC40-111-530-F
Description: Based on observation, the provider did not ensure that no soft bedding of any kind shall be used under or around infants including, but not limited to, pillows, quilts, comforters, sheepskins, or stuffed toys.
Evidence:
The inspector observed an infant sleeping on top of a pillow located on a couch. The manufacturers instructions warned consumers to never allow infants to sleep on the pillow and to not use on an elevated surface.

Plan of Correction: I will talk to the parents this evening and remove the pillow from the home.

Standard #: 22VAC40-111-540-A
Description: Based on observation, the provider did not ensure that infant carrier seats, swings, strollers, feeding or activity tables, and high chairs were used according to the manufacturer's instructions and when occupied by a child, a safety strap shall be used and securely fastened.
Evidence:
1. The inspector observed child #1 (3 months) in a swing, the provider did not ensure the safety strap was used and securely fastened.
2. The inspector observed two children eating in high chairs, the provider did not ensure the safety straps were used and securely fastened.

Plan of Correction: From this day forward the straps will be connected to the chair and straps will be used.
