Comments:
An unannounced monitoring visit and home tour was conducted from 10:30am-11:20am. At the time of inspectors' arrival there were 8 children in care ranging from 1 month-5 years of age, totaling 28 points in care with the provider and assistant provider. Children were observed participating in free play, and interacting with the provider and assistant provider through out the duration of the inspection. A sample of 5 children's and the provider's and assistant's records were reviewed. No medications are administered by the provider. There have been no injuries since the last inspection. Areas of noncompliance are detailed in the Violation Notice and were discussed with the provider at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on record review and interview it was determined the provider did not practice emergency evacuation procedures monthly.
Evidence:
1. There was no documentation of evacuation drill practices in November and December 2019.
2. The provider confirmed that with the busy holiday schedule and inclement weather they did not complete evacuation drills in November and December.

Plan of Correction: Provider will be sure to practice evacuation drills monthly.

Standard #: 22VAC40-111-100-A
Description: Based on record review and interview it was determined the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. There was no documentation of a physical examination for child #5(enrolled 11/18/19).
2. Provider confirmed there was no physical examination documentation for child #5.

Plan of Correction: Provider requested physical examination documentation from the parent during the inspection.

Standard #: 22VAC40-111-60-A
Description: Based on record review and interview it was determined the provider did not have a record for each child enrolled.
Evidence:
1. There was no record for child #5(enrolled 11/18/19).
2. The provider confirmed there was no record for child #5.

Plan of Correction: Provider requested documentation for the record from parent during the inspection.

Standard #: 22VAC40-111-90-A
Description: Based on record review and interview it was determined the provider did not obtain documentation that the child has been adequately immunized before attending the family day home.
Evidence:
1. There was no immunization documentation for child #5(enrolled 11/18/19).
2. The provider confirmed they did not have immunization documentation for child #5.

Plan of Correction: Provider requested required documentation from the parent during the inspection.

Standard #: 22VAC40-111-90-C
Description: Based on record review and interview it was determined the provider did not obtain documentation of additional immunizations every six months for children under the age of two years.
Evidence:
1. There were no updated immunization records for child #1 and child #4.
2. The provider confirmed that there was no updated immunization documentation in the record for child #1 and child #4.

Plan of Correction: The provider will request updated immunization documentation from the parents at pick up.
