Comments:
An unannounced renewal inspection was completed on 01/14/2020 from 11:05 A.M.-2:00 P.M. At the time of the inspection there were twelve children, two caregivers, and two visiting children with a guardian present. There was a total of nineteen points. The records for five children, two caregivers, one household member, three board members background checks, four injury reports, first aid kit and emergency supplies, outside play area, and required posted information were reviewed. The children were observed during circle time, music and movement activities, transition to outside play time, outdoor playtime, storytime, handwashing, lunchtime, transition to naptime, and quiet activities that included reading books and knitting. The exceptions to compliance are noted on the violation notice.

Violations:
Standard #: 22VAC40-111-830-B
Description: Based on a review of shelter-in-place drills, the provider failed to ensure that two shelter-in-place drills were conducted at a minimum of twice a year.

Evidence:
1. There was only one shelter-in-place drill conducted and documented for 2019. The only shelter-in-place drill conducted was dated 10/21/2019.
2. The provider verified that there was only one shelter-in-place drill conducted in 2019.

Plan of Correction: The provider will plan to complete two shelter-in-place drill for the fiscal year 2020.

Standard #: 22VAC40-111-100-A
Description: Based on review of children files, the provider failed to obtain a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

Evidence:
1. Child #5's file did not contain documentation of a physical examination. Date of enrollment for Child #5 was 07/01/2019.
2. The provider verified that there was not documentation of a physical examination completed.

Plan of Correction: The provider will contact the parent at pick-up today and request a copy of the physical examination.

Standard #: 22VAC40-111-90-A
Description: Based on review of children's records, the provider failed to obtain documentation that a child had been adequately immunized before the first day of attendance.

Evidence:
1. Child #1's immunizations were dated 07/24/2019. Date of enrollment for Child #1 was 07/08/2019.
2. Staff verified the date of enrollment and the date of the immunizations.

Plan of Correction: The provider will create and implemented a tracking form for each new enrollment to ensure all paperwork is received in the required time frame.
