Comments:
An unannounced monitoring inspection was conducted today between 9:30 am and 12:00 pm. There were 9 children (24 points) present with 3 caregivers. The child/caregiver interaction were very positive. The children were observed working on building with blocks, during diaper change, hand washing and eating lunch. Lunch consisted of Falafel, pita bread, peas and carrots, fruit and milk. The physical plant, children's records, caregiver's records, household member's records, evacuation drills, emergency plans, cribs, bottles and emergency supplies were inspected. Description: Based on a review of caregiver records, the provider failed to obtain a copy of the results of a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years.

Evidence:
Caregiver # 2's record does not have documentation of the results of a search of the central registry and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years.

Plan of Correction: I will submit form to the state the caregiver previously lived.
