Comments:
Today an unannounced Renewal inspection took place between 9:15am and 11:20am. There were 9 children present with the provider and 1 assistant for a total of 24 points within the supervision guidelines. The children were observed finishing breakfast, being dropped off by their parents, participating in a circle time and playing freely. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC4-665-340-I
Description: Based on observation and interviews, pillows were used by children under two years of age.
Evidence: Child #1 was observed in a crib with a pillow. The provider confirmed that the child was 6 months old.

Plan of Correction: This was corrected during the inspection.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #3's record states that they have a severe food allergy to multiple foods. The child's registration form does not contain actions to take in an emergency situation.
2. Child #2, Child #3 and Child #4's records do not contain a limited liability declaration.

Plan of Correction: I'll have the parents fill out the missing forms tonight. I'll have the parents fill in the emergency action.

Standard #: 22VAC40-111-240-B
Description: Based on observation, equipment that has been recalled was used.
Evidence: Child #2 was observed sitting in a bumbo chair. Bumbo chairs were recalled in 2012 unless a safety strap is obtained and attached to the chair to keep children from falling out. There was no safety strap attached to the providers bumbo chair.

Plan of Correction: I won't use it.

Standard #: 22VAC40-111-600-B
Description: Based on observation, an infant spent more than 30 minutes of consecutive time during waking hours confined. The intervening time period between confinements was not at least one hour.
Evidence: At the start of the inspection (approximately 9:15am) Child #1 was observed laying awake in their crib. At approximately 10:00am Child #1 was moved to a play chair until the end of the inspection, at approximately 11:20am.

Plan of Correction: We'll start holding her more and getting on the floor with her.
