Technical Assistance:
Reviewed with the provider how to clearly indicate the children's walking trip destination on the authorization form.

Comments:
An unannounced monitoring inspection was conducted today. The provider was caring for 6 children totaling 14 points. Children's records, the physical plant, children's nutrition, administration documents, emergency preparedness items, and childcare interactions were inspected today.

The children were observed eating morning snack, playing with toys,and interacting with the provider.

This inspection was completed between approximately, 10:15am-12:20pm. Thank you for meeting with me today, please email me with any questions.

Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation, various areas of the home have not been maintained in a safe condition.
Evidence: The inspector observed the following:
1. Two remote control batteries were on the floor in the downstairs childcare area.
2. The provider's purse was on the floor in the upstairs childcare area.The inspector observed two children trying to obtain items out of the purse.
3. In the downstairs childcare area, two tv cords were stretched between the tv and the wall outlet.These cords were long enough to pose a safety hazard to the children.
4. In the bathroom adjacent to the downstairs childcare area, the inspector observed adult mouthwash, shampoos and conditioners on the counter and inside the shower. These items were all accessible to the children.

Plan of Correction: 1. The batteries were immediately removed during the inspection.
2. The purse was immediately removed during the inspection.
3. The cords will be removed this evening.
4. The bathroom door will be locked with a child safety lock.

Standard #: 22VAC40-111-320-B
Description: Based on observation, one electrical outlet did not have a protective cover.
Evidence: In the downstairs childcare area, one electrical on the bottom half of the wall was not covered with a protective cover.

Plan of Correction: The outlet will be covered today.

Standard #: 22VAC40-111-540-A
Description: Based on observation, the provider allowed children to be seated in high chairs without the safety strap securely fastened.
Evidence: The inspector observed four children seated in high chairs eating morning snack. These children were not seated with the safety strap securely fastened.

Plan of Correction: I will remember to buckle the safety straps when children are seated in high chairs.
