Comments:
An unannounced monitoring inspection was conducted today from 11:00am-12:45pm. There were 10 children (24 points) supervised by 2 direct care staff. The home, 2 staff records, 7 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed watching a video, playing with trucks, trains, Legos, dinosaur eggs, and having storytime. Handwashing procedures were also observed. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 4 of 7 children's records did not contain the following information:

Evidence:

1. Child #4's (age: 2 years old) record did not contain documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B. The most recent documented parental review was dated 1/7/2019.
2. Child #6's (age: 2 years old) record did not contain documentation of the review of the child's emergency contact information as required by 22 VAC 40-111-780 B.
3. Child #7's (age: 2 years old) and Child #9's (age: 18 months old) records did not contain documentation of proof of identification.

Plan of Correction: We will get papers signed today and the birth certificate information by Monday.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not ensure that children's hands are washed with liquid soap and warm running water before eating.

Evidence: The children in care did not wash their hands before eating lunch.

Plan of Correction: We will be sure to wash children's hands before lunch moving forward.
