Provided a copy of the updated sworn statement.
Discussed transitioning the 2 year old out of the crib and onto a mat. The crib sheet that the toddler uses for nap needs to be fitted to the mattress.

An unannounced monitoring inspection was conducted today between the hours of 8:45 am through 10:15. There were a total of 5 children (11 points) observed with 1 caregiver. A total of 3 children's files and 2- staff/household member's files were reviewed. The children were engaged in the following: toy play, blocks and trucks. Standard #: 22VAC40-111-60-B Description: Based on a review of children's records, each child's record does not contain all the required information.
Evidence: Child # 3, date of first attendance 6/10/19 does not have documentation of a second emergency name, address and phone number.Plan of Correction: I will ask the parent for the information.
Standard #: 22VAC40-111-90-A Description: Based on a review of children's records, before a child may attend the provider has not obtained documentation of children's immunization record.
Evidence: There is no documentation of immunization record for child #1 and child #2, both first day of attendance 6/17/2019.Plan of Correction: Will get form from doctor.
