Comments:
An announced initial inspection was conducted with the provider on January 13, 2020 beginning 9:00 AM and ending at 11:00 AM. There were four children in care during the inspection. The children's ages were 2, 2, 2, and 3 years old for a total of 8 points. The children were observed dancing and singing to an age appropriate television program and playing with blocks. The children were also observed eating breakfast which consisted of cereal, bananas, and milk. Two household records were reviewed and in compliance. Five children's records were reviewed. The first aid kit, flashlight, and weather-band radio were observed and in compliance. There were no medications on site to be reviewed. The provider has current First Aid and CPR. The provider is also Medication Administration Training (MAT) which is required to administer prescription medication. Not all of the standards were able to be assessed during the initial inspection.

Standard #: 22VAC40-111-100-A
Description: Based on a review of documentation, the provider did not obtain documentation of a physical examination for all children within 30 days after their first day of attendance. Evidence: Child A (start 2/4/19), Child B (start 8/19/18), Child C (start 8/26/19), and Child F's (start date 5/14/18) records were missing documentation of a physical examination

Plan of Correction: The provider will request the missing information from the parents. The children's records will be updated with the missing information.

Standard #: 22VAC40-111-60-B
Description: Based on a review of documentation, not all of the children's records contained the required information. Evidence: Child A's record was missing the medical insurance policy number and physical address for emergency contacts. Child B's record was missing the home address and phone number, parents' employment contact information, two emergency contact information, medical insurance company name and policy number. Child C's record was missing medical insurance company name and policy number. Child D's was missing a second emergency contact person. Child E's record was missing phone number for one emergency contact.

Plan of Correction: The provider will request the missing information from the parents. The children's records will be updated with the missing information.

Standard #: 22VAC40-111-80-A
Description: Based on a review of documentation, the provider did not obtain proof of age and identity for all children enrolled in program within 7 business days of the child's first day of attendance. Evidence: Child A (start 2/4/19), Child B (start 8/19/18), and Child F's (start 5/14/18) records were missing documentation of proof of age and identity.

Plan of Correction: The provider will request the missing information from the parents. The children's records will be updated with the missing information.

Standard #: 22VAC40-111-90-A
Description: Based on a review of documentation, the provider did not obtain documentation of immunization for all children prior to their first day of attendance. Evidence: Child A (start 2/4/19) and Child F's (start 5/14/18) records were missing documentation of immunizations.

Plan of Correction: The provider will request the missing information from the parents. The children's records will be updated with the missing information.
