Technical Assistance:
Had a discussion with the provider regarding prescription medication that was observed in the program's refrigerator. Reminded the provider that they cannot administer medication to an enrolled child without being MAT trained and without the parents written authorization to administer.

Comments:
An unannounced monitoring inspection was conducted today between 11:15am and 1:45pm. There were 11 children present with the Provider and 1 assistant for a total of 31 points within the supervision guidelines. The children were observed dancing to music, working on letter M and O worksheets, coloring, playing with kitchen toys, magnets, eating lunch and napping. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-960-E
Description: Based on observation, bottlers were not labeled with children's full name and the date.
Evidence:
1. 8 out of 8 bottles in the refrigerator did not contain today's date.
2. 2 out of 8 bottles in the refrigerator did not contain a child's name.

Plan of Correction: I always leave it up to parents to label the bottles and if the labels come off they haven't been doing it. I'll be more strict about labeling.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider has not obtained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #1 (date of enrollment: 2/25/2019) file does not contain a physical exam.

Plan of Correction: I'll ask the parents to provide this.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence:
1. Child #1's file does not contain the families medical insurance policy number.
2. Child #7's (date of enrollment: 1/6/2020) file does not contain documentation that the child's age and identity has been verified.
3. Child #2, Child #3, Child #4, Child #5 and Child #6's files did not contain documentation of the review of the children's emergency contact information.

Plan of Correction: I will ask the parents and make sure everything gets reviewed and I'll get the missing info from the parents.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation of immunizations before each child could attend the family day home.
Evidence: Child #8's (date of enrollment: 1/13/2020) file does not contain documentation of immunizations. The child was present during today's inspection.

Plan of Correction: I'll ask the parents to provide this.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregivers did not ensure that children's hands were washed.
Evidence: Child #8 was observed having their diaper and soiled clothes changed. The child's hands were not washed with running water or a disposable wipe after the diaper change was complete.

Plan of Correction: We'll remember to do it.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diaper changing surface is not nonabsorbent or sanitized after each use.
Evidence: The diaper changing surface has a soft, fabric cover with a plastic strip down the middle where the children lay. The entire surface is not non-absorbent and cannot not be adequately sanitized after each use.

Plan of Correction: We will remove the cover.
