Comments:
An unannounced Renewal inspection was conducted this date. A total of 11 children (31pts) with Provider and 1 Assistant was on site. The purpose of this inspection was to evaluate compliance during the six month Provisional licensure period. Some repeated systemic/uncorrected issues were found today. Walk thru of the home was made. The children were observed watching a program on the TV, free playing in small groups or independently, sliding down the slide with staff supervision and reading books. The provider was encouraging and assisting infants with walking. Hand washing procedures were also observed. No new children enrolled since the October inspection, however some recordation issues remain. An acceptable corrective action for each violation found today will be forwarded to the Licensing Inspector for review. A recommendation for issuance of a new license will remain pending at this time. This inspection began at approximately 9:20am and ended at 11:30am. Standard #: 22VAC40-111-830-A
Description: Based on review of Provider's emergency evacuation drill log, monthly fire evacuation drills have not been practiced monthly.
Evidence: The last fire evacuation drill was completed in July 2019. No documented evacuation drill was conducted for the months of August, September, October, November and December.

Plan of Correction: We will conduct a fire evacuation drill today. And will forward the documented drill to the Licensing Inspector.

Standard #: 22VAC40-111-60-B
Description: Based on child record review, 2 children's records did not contain all required information.
Evidence:
1. Child #2's record did not contain addresses for two emergency contacts and a signed acknowledgement of parent information.
2. Child #3's record did not contain two emergency contacts, medical insurance policy number and authorization for emergency medical care.

Plan of Correction: I will have this information and will forward the information to the State Inspector by Friday.

Standard #: 22VAC40-111-200-A
Description: Based on staff record review, one staff has not received orientation.
Evidence: Staff #2 does not have documented completion of orientation training. This remains uncorrected during the July and October licensing inspection.

Plan of Correction: This staff will be trained and will forward documented completion of training to the Licensing Inspector by tomorrow.

Standard #: 22VAC40-111-430-C
Description: Based on observation, Children over the age of 2 years have access to stairs without a protective barrier.
Evidence: The stairs leading to the second level of the home was accessible to children over the age of 2 years and did not have a gate/barrier. The gate was observed leaning against the wall at the base of the steps.

Plan of Correction: The gate is broken. We will either purchase a new gate or will place a barrier to prevent access. Will forward a picture of the correction to the Licensing Inspector.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation of the home, the findings of the most recent inspection was not posted.
Evidence: The October findings from the licensing inspection was not made available.

Plan of Correction: I will post today's findings.
