Technical Assistance:

Reviewed the required background checks and timeframe for completion.

Comments:

An unannounced renewal inspection was completed today. The physical plant, 6 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 2:45-4:10pm. There were 11 children, totaling 22 points, under the supervision of 2 caregivers. Today, the children exercised, completed a lesson on numbers and letters in the Russian alphabet, listened to storytime and took a nap. The interactions between the children and caregivers were seen to be positive and constructive in nature. For lunch, the children were served meatball and vegetable soup, buckwheat,chicken nuggets, bread and milk. There are no caregivers certified in MAT (medication administration training) at this time. Description:	Based on observations, the emergency evacuation drill log does not have documentation of drills being completed each month that children receive care. Evidence 1. There most recent documentation of an evacuation drill having been completed was on November 20, 2019.
Plan of Correction:	These will be completed and recorded each month.
Standard #:	22VAC40-111-100-A
Description:	Based on file review, 1 out of 5 children's records does not have a physical examination. Evidence 1. Child #5 does not have a physical on file.
Plan of Correction:	I will ask the parents for this again.
Standard #:	22VAC40-111-60-B
Description:	Based on file review, 2 out of 5 children's records do not have all required information. Evidence 1. Child #4 does not have the name, address and phone number of 2 emergency contacts in case the parents cannot be reached. 2. Child #5 does not have the start date of receiving care at the FDH.

Description:	Based on file review, 1 out of 5 children's records does not have documentation that the child has been adequately immunized before attending the FDH. Evidence 1. Child #5 does not have an immunization record on file.

Description:	Based on file review, 1 out of 6 caregivers do not have additional TB test/screenings completed every two years. Evidence 1. Caregiver #3 has a TB test which was last completed on July 5, 2017.
