Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and documentation obtained today for a TB test. Due to the Provider obtaining this information for herself and another household member, the Provider had scheduled a late opening for the Family Day Home (FDH). Discussion was also held about possibly registering a fictitious name "Second Home" for her FDH with the Virginia State Corporation Commission (SCC). The Provider stated that she may do this at another time, but will continue to label the business in her name.

Comments:
The following inspection took place between the hours of 9:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Herndon, Virginia. At the start of this unannounced renewal inspection there were a total of 1 child observed (2 points) with 1 caregiver. A total of 3 children files, 1 staff file, and 1 household member?s file was reviewed. The child was engaged in the following: drop off, morning snack, song, learning, toy play, and other organized activities. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review, 2 of 3 children files indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days after the first day of attendance.

Evidence -
1. On the date of inspection (01/16/2020), documentation of a physical examination was not available for review for the following children: Child #1 and Child #3.
2. The start date for each child were listed as - 06/03/2019 for Child #1, and 01/07/2019 for Child #3.
3. This same standard (for these children) was cited previously during an inspection conducted on 02/15/2019 for Child #3, and again on 08/16/2019 for Child #1 and Child #3.

Plan of Correction: All missing documentation of a physical examination shall be obtained for all applicable children.

Standard #: 22VAC40-111-70-A
Description: Based on review, 2 of 3 children files indicated that not all of the required documentation was contained within each child's file.

Evidence -
1. On the date of inspection (01/20/2020), a signed Policy for the Administration of Medication for the Family Day Home (FDH) form was not available for review for the following children: Child #2, and Child #3.
2. On the date of inspection, the Provisions of the Emergency Preparedness and Response Plan form was not signed by the parent of Child #2.
3. This same standard for these children was cited previously during an inspection conducted on 02/15/2019, and again on 08/16/2019.

Plan of Correction: All missing documentation shall be completed, signed, obtained, and placed on file.
