Comments:
An unannounced monitoring inspection was conducted today from 10:10am-11:30am. There were 9 children (21 points) directly supervised by 3 staff. The home, 2 staff records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed reading books and doing circle time together with a caregiver, preschool age students were building with legos, writing letters, reading their books along with a caregiver, and gluing numbers on to a paper in order. The provider worked with children individually to sound out words and reading their own personal books. Infants and toddlers were observed completing chunky puzzles and playing with large blocks. Caregivers talked to children in quiet and even tone. Children were excited to do the work and were laughing and playing with caregivers. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation, medications were not kept in a locked place preventing access by children.

Evidence: Medication was observed in the door of the fridge. The fridge was not locked.

Plan of Correction: The medication was removed and brought upstairs as the medication belonged to a household member.

Standard #: 22VAC40-111-60-B
Description: Based on observation, children's records did not contain all the required information.

Evidence: Child 1 does not have a second emergency contact listed.

Plan of Correction: I will contact parents and have them complete a second emergency contact.

Standard #: 22VAC40-111-70-A
Description: Based on observation, before the child's first day of attendance parents were not informed in writing about all required information.

Evidence: Child 1's guardians did not have documentation of being notified of the family day home's emergency preparedness plan.

Plan of Correction: I will give parents the document and have them sign it.
