Comments:
An unannounced monitoring inspection was conducted today approximately 10:40 am- 12:30 pm. There were 5 children, totaling 16 points, supervised by 2 direct care staff. The physical plant, 2 staff records, 3 children?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Medications were on-site today and records were reviewed. The playground and surfacing were inspected. The children were observed in free play and in center activities. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Standard #: 22VAC40-111-800-A
Description: Based on staff interview and observation, the family day home did not have written emergency preparedness and response plan.
Evidence: The provider did not have documentation of the written emergency preparedness and response plan.

Plan of Correction: Will work today.

Standard #: 22VAC40-111-170-A
Description: Based on household records review, the provider did not obtain from each caregiver at the time of hire and each adult household member prior to coming into contact with children a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Evidence: 1. Household member #1 moved in the home in December and did not have documentation available for review in record of a current Report of Tuberculosis Screening.

Plan of Correction: Will get a TB test

Standard #: 22VAC40-191-40-C-1-c
Description: Based on household records, the provider did not obtain background checks for other adults living in the home.
Evidence: Household member #1 did not have initial background checks. This household member moved in the home in December 2019.

Plan of Correction: Will complete background checks.
