Comments:
An unannounced renewal inspection was conducted today approximately 10:40 am- 1:30 pm. There were 3 children supervised by 2 direct care staff. The physical plant, 2 staff records, 1 child?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Medications were on-site today and records were reviewed. The children were observed in free play and in center activities. Diapering changing and hand washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Violations:
Standard #: 22VAC40-111-230-B
Description: Based on staff record review, the provider did not obtain 16 hours of annual training.
Evidence: Staff #1 and #2 had no documentation training in the last year.

Plan of Correction: Will send documentation of training hours

Standard #: 22VAC40-111-240-A
Description: Based on observation, areas and furnishings of the family day home, inside were not maintained in a safe and operable condition.
Evidence:
1. In the lunch room located by the door entrance, there was peeling wall paint.

Plan of Correction: Will removed it and sand the wall area.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diaper surfacing was not nonabsorbent.
Evidence: The changing table cover had two small tears which is unable to be cleaned and sanitized after each use.

Plan of Correction: Will get a new on Saturday.
