Comments:
An unannounced monitoring inspection was conducted today between the hours of 11:20 am through 12:50 pm with the provider on site. There were 9 children with the provider and two assistants. A sample of 4 children's files and 4 caregiver/household member's files were reviewed. The children were observed playing with toys, building with blocks, and preparing for lunch. The physical space, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 4 children's records did not contain all required information.

Evidence:
1. The record for Child #1 did not contain documentation of telephone number of the mother's place of employment.
2. The record for Child #1 did not contain names of persons other than the custodial parents who are authorized to pick up the child

Plan of Correction: I will have the parent fill out the work number and authorized pick up.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain, for one adult household member, a current Report of Tuberculosis Screening form every two years from the date of the first screening.

Evidence:
The most recent documentation of a tuberculosis screening in the record for Household Member #1 was dated 8/4/17.

Plan of Correction: They have received an updated tuberculosis screening. I will get the paperwork.

Standard #: 22VAC40-111-350-E
Description: Based on observation, hot water at taps available to children were not maintained within a range of 105?F to 120?F.

Evidence:
Hot water from the bathroom sink , accessible to children, measured at 125?F during the inspection.

Plan of Correction: I will set the water temperature for 120 degrees.
