Technical Assistance:
Discussed with provider requirements for household members and recordkeeping for non-resident relative children. Discussed with provider that a renewal application must be received prior to license expiry date (2/27/2020) in order to renew the license for the family day home.

Comments:
An unannounced monitoring inspection was conducted today between the hours of 10:40 am through 2pm with the provider on site. There were 5 children enrolled with 2 resident children (16 points) with the provider. A sample of 4 children's files and 2 caregiver/household member's files were reviewed. The children were observed during snack time, circle time, reciting nursery rhymes, singing and dancing, playing with toys, and naptime. Handwashing and diapering were also observed. The physical space, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-730-D
Description: Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children.

Evidence:
Prescription medication for a household member was observed in an unlocked cabinet located in the kitchen.

Plan of Correction: A lock was placed on the cabinet door during inspection.

Standard #: 22VAC40-111-50-A
Description: Based on record review, the family day home has not kept a written record of children in attendance each day.

Evidence:
The most recent daily attendance recorded at the family day home was dated 01/09/2020.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-60-A
Description: Based on record review and caregiver interview, the provider did not maintain an up-to-date record for one child.

Evidence:
Child #1 began attending the family day home on 01/15/2020. There was not a record available for review for Child #1. Provider stated Child #1 was a family member, but is not a resident child.

Plan of Correction: I will have a file for him.

Standard #: 22VAC40-111-90-A
Description: Based on record review and caregiver interview, the provider did not obtain documentation that one child has been adequately immunized before attending the family day home.

Evidence:
Child #1 began attending the family day home on 1/15/2020. Provider stated Child #1 is a family member but is not a resident of the family day home. There were no immunization records for Child #1 available for review during inspection.

Plan of Correction: I will ask the parent today for the immunizations.

Standard #: 22VAC40-111-280
Description: Based on observation, potentially poisonous substances were not stored in areas inaccessible to children.

Evidence:
5 cans of paint were observed on the floor outside of the bathroom, accessible to children.

Plan of Correction: I will take it upstairs.

Description: Based on observation, the bathroom did not contain all required items.

Evidence:
The bathroom did not contain liquid soap.

Plan of Correction: Liquid soap was placed in the bathroom during inspection.

Standard #: 22VAC40-111-350-E
Description: Based on observation, hot water taps available to children were not maintained within a range of 105?F to 120?F.

Evidence:
Hot water from the bathroom sink, available to children, measured 126.2 degrees F.

Plan of Correction: I will ask my son to help me change the hot water temperature.

Standard #: 22VAC40-111-540-A
Description: Based on observation and caregiver interview, infant high chairs were not used according to manufacturer's instructions.

Evidence:
2 of 4 children were observed not securely fastened with a safety strap while sitting in a high chair.

Plan of Correction: Provider stated the children were securely fastened but unfastened themselves. The children were removed from the high chairs.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregiver did not ensure that one child's hands were washed after diapering.

Evidence:
One child's hands were not washed after a diaper change.

Plan of Correction: Corrected during inspection.
