Comments:
An unannounced renewal inspection was completed on Thursday, January 16, 2020 from approximately 11:40am to 1:55pm to determine the provider?s compliance with licensing standards. Upon arrival, the children were observed in the designated area for childcare watching television. Later, the children were observed during free-play, interacting with peers and the providers. The designated areas for childcare were deemed appropriate and equipped with age appropriate toys and materials to meet the needs of the children. During the inspection, the licensing inspector interacted with staff and children when appropriate. The bathroom, kitchen, and designated childcare areas were all inspected during today's inspection. The inspector observed the required postings to include license, prior inspection summary, evacuation mapping. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were inspected. The outdoor space is currently not used. Five children?s records, two caregiver records, and one household member's record were reviewed today. There were no injury reports to review. The provider does not administer medications or provide transportation at this time. The most recent evacuation drill was documented on 01/03/2020. The most recent shelter-in-place drill was documented on 01/03/2020.

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of five children's records on 01/16/2020, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to the child's attendance or within 30 days after the first day of attendance.

Evidence: 1) The record for Child #5, enrolled on 12/12/2019, did not contain the child's physical examination. 2) During interview, the provider reported the physical examination for Child #5 has not been obtained to date.

Plan of Correction: The provider reported Child #5 will be going to the doctor next week. A copy of the most recent immunizations will be obtained at that time and kept on file for the child.

Standard #: 22VAC40-111-30-A
Description: Based on observation and interview on 01/16/2020, the provider did not ensure compliance with these standards and the terms of the current license issued by the department and with relevant federal, state or local laws, and other relevant regulations.

Evidence: 1) The licensing inspector observed that the provider's zoning permit requires a conditional use permit for any family day home that employs persons from outside the home. 2) During interview, Provider #1 reported she does not have a conditional use permit from zoning to authorize the provider to employ a caregiver who is not a resident of the home. Provider #2 reported she does not reside in the home.

Plan of Correction: The provider will be in contact with zoning to review and obtain a conditional use permit to employ a person that does not reside in the home.

Standard #: 22VAC40-111-90-A
Description: Based on a review of five children's records and interview on 01/16/2020, the provider did not obtain documentation that one child has been adequately immunized before the child attended the family day home.

Evidence: 1) The record for Child #5, enrolled on 12/12/2019, contained immunization documentation that was dated 01/08/2020. 2) During interview, Provider #2 reported there was no prior immunization documentation obtained for Child #5.

Plan of Correction: The provider reported Child #5 will be going to the doctor next week. A copy of the most recent immunizations will be obtained at that time and kept on file for the child.

Standard #: 22VAC40-111-210-A
Description: Based on a review of provider records and interview on 01/16/2020, the one provider did not obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.

Evidence: 1) The record for Provider #1 did not contain documentation of annual training hours for 2019. 2) During interview, Provider #1 reported she only completed a total of approximately 10 hours, but documentation could not be located while the inspector was on site. In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.

Plan of Correction: The provider will complete the missing hours and will obtain the additional 16 hours of annual training for this year.
