Comments:

An unannounced monitoring inspection was completed today. The physical plant, 1 caregiver/household member files, 3 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 1:40-2:45pm. There were 3 children, totaling 6 points, under the supervision of the Provider. Today, the children had circle time, learned the ABC's in English and Spanish, listened to music, enjoying free play and resting during naptime. For lunch, the children were served beef, rice, cucumber, banana and milk. The Provider is not certified in MAT (medication administration training) at this time. Description:	Based on file review, 3 out of 5 children's records do not have all required information. Evidence 1. Child #3 does not have a complete record. The only information that the Provider has on this child is the immunization record. All other paperwork needs to be completed.
Plan of Correction:	The Provider will give the paperwork to the parent today to have it completed.
Standard #:	22VAC40-111-90-C
Description:	Based on file review, 1 out of 3 children's records reviewed does not have additional immunizations every six months up to the age of 2 years. Evidence 1. Child #2 is 2 years and 10 months old. The most recent immunization record was updated when the child was 15 months old.
Plan of Correction:	The Provider will ask the parents for the 2 year old shot record.
Standard #:	22VAC40-111-180-A
Description:	Based on file review, the Provider does not have a TB test/screening completed within the past two years. Evidence 1. The Provider's most recent TB test/screening was completed on 1/19/2018.
Plan of Correction:	The Provider will have this completed again.
Standard #:	22VAC40-111-340-C
Description:	Based on observations, the Provider failed to keep all required items in the bathroom. Evidence 1. In the bathroom, there were no paper towels.
