An unannounced renewal inspection was conducted on December 17 2019 between 8:45am and 10:00am. There were 6 children in the direct care of 2 staff members. Children were observed eating a snack and playing with toys. The home was inspection as well as staff records, children's records, household records, the emergency plan, evacuation documentation, and first aid kit. Description:	Based on record review, 4 of 4 children's records did not contain all of the required documentation signed by parents before the child's first date of attendance. Evidence: Child 1, 2, 3, and 4 records have documentation of liability insurance, but are missing medication forms, information for parents, and emergency preparedness information.

Description:	Based on observation, the provider did not ensure all cribs, that are used with infants, are equipped with tight-fitting bottom covers. Evidence: A crib in the day care contained a mattress with a sheet that could be easily manipulated by a child.
