Technical Assistance:
Discussion was held regarding attendance records and required training. Licensing and subsidy standards require that written attendance is recorded daily for all children attending the family day home (FDH). Subsidy standards also require that the arrival and departure of children be documented daily. These records are to be kept available for review in the FDH. New assistants must complete the Preservice training withing 90 days from hire.
VDSS Licensing Inspectors will require existing Subsidy Vendors and staff to have a certificate of completion for The Virginia Annual Health and Safety Update Training on file by March 1, 2020.

Comments:
A Subsidy Health and Safety inspection was conducted on 01/21/2020 to review supplemental health and safety requirements. The subsidy requirements were reviewed this date; staff trainings, nutrition and food services, recordkeeping, hazardous substances and emergency plans. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Standard #: 22VAC40-665-160-B
Description: Based on documentation review and interview, the provider did not maintain a written hard copy record of daily attendance that documents the arrival and departure of each child in care as it occurs.
Evidence:
1) a written attendance record was not available for review for the date of inspection nor any other dates.
2) The provider showed a form that she intends to use for taking attendance and stated that she forgets to take attendance.

Plan of Correction: I will start taking attendance daily.

Standard #: 22VAC40-665-410-A-3
Description: Based on documentation review and interview, it was determined that lockdown procedures were not practiced at least annually.
Evidence:
1) On the date of inspection, the emergency response drills log did not contain any documentation of a lockdown drill being conducted for the year 2019.

Plan of Correction: I will practice the lockdown drill and make sure to document it.
