Technical Assistance:
Discussed substitute provider records.

Comments:
An unannounced monitoring inspection was conducted today from 12:00pm-2:30pm. There were 9 children (28 points) supervised by 2 direct care staff. The home, 2 staff records, 5 children?s records, 5 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed eating lunch, engaging freely, and transitioning to naptime. There was a sufficient amount of books, toys, and materials, which were found to be developmentally appropriate for the children in care at the time of inspection. Areas of non-compliance are identified in the Violation Notice.

Violations:
Standard #: 22VAC40-111-770
Description: Based on observation, the provider did not maintain a emergency flashlight and radios.

Evidence: There was no flashlight.

Plan of Correction: I will go and buy one today.

Standard #: 22VAC40-111-60-B
Description: Based on record review, a child's record did not contain the following information:

Evidence: Child #1's (age: 5 months old) written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.

Plan of Correction: I will get the signature today during pick up.

Standard #: 22VAC40-111-180-A
Description: Based on record review, for each caregiver and adult household member, the provider did not obtain a current report of tuberculosis screening form every two years from the date of the first screening.

Evidence: Household member B's most recent documented TB screening was dated 9/11/2017 (expire: 9/11/2019).

Plan of Correction: I will have the individual go get it done this week.

Standard #: 63.2-1721.1-B-3
Description: Based on record review, the provider did not obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years.

Evidence: Household member C's Virginia central registry request indicated that the individual has lived in the state of Maryland within the past 5 years. There was no documentation of a central registry request for Maryland.

Plan of Correction: I will submit it this week.
