Technical Assistance:
We discussed the requirements for children's records and the annual review of the emergency plan. The risk assessment for violations were provided.

We talked about your plans for outside maintenance. Also, we talked about training due dates.

Comments:
Thank you for your assistance during the monitoring inspection conducted from 1:09 PM to 5:25 PM. Today, there were four school age children in care during before and and after school hours, with the provider. Four resident school age children have been regularly present. I viewed daily routines, menu, nutrition, staffing, supervision, interactions with children, pet record, posted information,activities, attendance record, equipment, activities, emergency supplies and drill records, emergency response plan, four records for children, one caregiver record, caregiver training, posted information, indoor and outdoor areas. Outside time has been enjoyed by the children, weather permitting. Nutritious meals have been provided each day. The provider has participated in available local training.

Standard #: 22VAC40-111-780-B
Description: Based on a review of four children's records, and an interview with the provider, the provider did not complete an annual review with all parents to ensure the written emergency contact information is correct. Evidence: The last parent signature on the registration form for child 1 was December 18, 2018. The parent of child 2 signed on December 19, 2018. The parent of child 3 signed on December 17, 2018. The provider indicated she did not have parents review the contact information during recent months.

Plan of Correction: The provider will have the parents look over the contact information in the records by Friday. The provider will have parents review the information again each year.

Standard #: 22VAC40-111-800-B
Description: Based on a review of the emergency response plan, and an interview with the provider, the provider failed to conduct an annual review of the plan and determine any updated procedures. Evidence: The section for documenting the date of the review and whether there were any changes to the emergency plan was not recorded since the new plan was signed on December 1, 2018. The provider confirmed she did not review the play by December 2019.

Plan of Correction: The provider immediately looked over the emergency plan and documented the review. The provider will review the plan by January 2021.

Standard #: 22VAC40-111-60-B
Description: Based on a review of four records for enrolled children, and an interview with the provider, the record for child 1 did not include all required information. Evidence: There were not at least two emergency contact names other than the parents, including addresses and phone numbers, for child 1. The provider indicated she did not have the written information for the record.

Plan of Correction: The provider will have the parent add information for two emergency contacts for child 1.
