Technical Assistance:
Technical assistance was offered regarding background checks for employees and household members.

Comments:
An unannounced inspection was conducted in response to a complaint received on 1/6/20. The inspection was conducted from 12:15 pm to 1:10 pm. There were 12 children (28 points) in care of 4 caregivers. The complaint was related to persons residing in the family day home without obtaining the required fingerprint results. 4 staff were interviewed and a walk through of the family day home was conducted. 4 caregiver records and 1 household member record was reviewed. Based on the preponderance of evidence, the complaint is not valid. Violations non-related to the complaint are listed in the Violation Notice. Description: Based on records review, the family day home did not obtain the sworn disclosure statement (SDS) by the first day of employment.
Evidence:
1. Caregiver # 1's first day of employment was 1/21/20.
2. Caregiver # 1's record did not have a complete SDS.

Plan of Correction: I will have the caregiver complete the form today and keep in here record.
