Comments:
The licensing inspector conducted an unannounced monitoring inspection on Thursday, January 23, 2020 from approximately 11:00 AM until 1:30 PM to determine the provider?s compliance with licensing standards. There were 11 children in attendance being care for by two caregivers. Based on the children?s ages there was a total of 29 points. Upon the inspector?s arrival, the children were observed in the designate area for childcare coloring, infants being held, rocked and fed. Later, the children were observed taking bathroom breaks, washing hands, eating lunch, and preparing for nap time. The caregivers were observed having positive interactions with the children. The designated areas for childcare were deemed appropriate and are equipped with age appropriate toys and materials to meet the needs of the children. During the inspection, the licensing inspector interacted with staff and children when appropriate. The bathroom, kitchen, and designated childcare areas were all inspected during today's inspection. The inspector observed the required postings. The first aid kit and emergency supplies, posted emergency contacts, fire and shelter-in-place drills were inspected. Five children?s records, three caregivers? records, and one household member?s records were reviewed during the inspection. Medication is administered, but none on site at the time of the inspection. Transportation is not provided at this time. The most recent evacuation drill was documented on 12/11/2019. The two most recent shelter-in-place drills were documented on 03/25/2019 and 11/20/2019. A lockdown drill was documented on 12/16/2019.

Violations:
Standard #: 22VAC40-111-170-B
Description: Based on a review of records on 01/23/2020, the provider did not ensure that one caregiver had a report of tuberculosis (TB) screening completed within the required time frame.

Evidence: 1) The TB results in the record for Caregiver #3 (DOH: 12/18/2019) were dated 08/19/2019. The form shall have been completed within the last 30 days and be signed by a physician, physicians' designee, or an official of the local health department.

Plan of Correction: Per the Provider: Caregiver #3 will submit a new TB screening form. In the future, TB results older than 30 days from hire date will not be accepted.
