Technical Assistance:
Discussion on children's records.

Comments:
An unannounced monitoring inspection was conducted today between the hours of
10:40 am through 1:30 pm with the provider on site. There were 3 children (8 points) with the provider and one assistant. A sample of 1 child's file and 2 staff member's files were reviewed. The children were engaged in the following: toy play and musical movement, hand washing, and other organized activities. Interactions between the children and caregivers were positive. The physical space, emergency supplies, evacuation drills, attendance records, injury reports, and the Emergency Preparedness Plan were reviewed. No medications today.

Description: Based on children record review, the provider did not keep a child record and make accessible to department's representative for five years after termination of services or separation.
Evidence:
1. The provider did not have a record for Child #1.

Plan of Correction: Will obtain and send copy to Licensing.
