Comments:
An unannounced monitoring inspection was conducted today, January 23, 2020 to determine the providers compliance with family day home standards. The inspector arrived at 2:00 pm and concluded the inspection at 5:00 pm. Upon the inspector's arrival there were two family day home children present. The provider's own two children were also in care. Later three additional family day home children and three resident children arrived from school for a total of eleven points with the one provider. Children were observed playing with cards, magnetic blocks, balls and on tablets. One child practiced writing his name for the inspector. Snack was observed with children washing their hands before and after eating. Today's snack consisted of apples, yogurt and milk. All areas used for child care were inspected including a walk through of the home. The family day home has age appropriate toys and materials in operable condition for children in care. The first aid kit and non-medical emergency supplies were reviewed. Required postings were reviewed including documentation of the emergency preparedness plan, menu, license, most recent inspection notice, attendance sheets, allergy list and emergency numbers. A smoke detector and fire extinguisher were observed. The inspector interacted with the provider and children in care.

One caregiver record and five children's records were reviewed.
Medication is not being administered. The provider has current first aid training.
The last documented emergency evacuation drill was held on 12/11/2019. The two required shelter-in-place drills were completed on 04/08/2019 and 05/19/2019. A lock down drill was documented on 06/04/2019.
Per the provider, transportation is not provided.
Violations previously cited were checked for corrections. There was one repeat violation during today's inspection.

Violations:
Standard #: 22VAC40-111-760-A-1
Description: Based on inspection of the first aid kit, the provider did not ensure the first aid kit contained all required items.

Evidence:

The first aid kit did not contain the required single use gloves.

Plan of Correction: Per the provider: I will purchase more gloves for the first aid kit.

Standard #: 22VAC40-111-60-B
Description: Based on review of five children's records, the provider did not obtain all required information for each child's record.

Evidence:

1. The record of child #1, enrolled 09/23/2019, did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.
2. The record of child #5, enrolled 09/23/2019, did not contain written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately.

Plan of Correction: Per the provider: I will have the parent complete the missing information tonight at pick up time.

Standard #: 22VAC40-111-140-B-3
Description: Based on review of documentation and interview, the provider did not maintain current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care.

Evidence;

1. The most recent documentation of CPR certification for caregiver #1 had an expiration date of 01/05/2020.
2. Caregiver #1 confirmed the certification in CPR had expired and had not been renewed.

Plan of Correction: Per the provider: I will schedule a class to renew my CPR. I thought my CPR was good for two years but it was only one year.

Standard #: 22VAC40-111-180-A
Description: Based on review of caregiver records, the provider did not obtain a current report of tuberculosis screening every two years from the date of the first screening.

Evidence:

1. The most recent tuberculosis screening for caregiver #1 was dated 10/08/2017, exceeding the requirement to renew every two years.
2. The provider stated she had not obtained a current tuberculosis screening in the past two years.

Plan of Correction: Per the provider: I will go tomorrow morning and get an updated tuberculosis screening.
