Description: Based on a review of records, one child's record did not contain documentation of what document was viewed for proof of birth verification.
Evidence: The child record showed a document was viewed: however, no notice of which form of documentation was viewed. (Child 1)

Plan of Correction: The parent has been asked to bring the birth certificate in again for documentation.

Description: Based on direct observation, a small appliance was plugged in while not in use.
Evidence:
A hair dryer was plugged in while not in use, in the upstairs master bathroom.

Plan of Correction: It was unplugged during the inspection. I will make sure to unplug the hair dryer daily.

Standard #: 22VAC40-111-320-I
Description: Based on a provider's statement, there is no fire extinguisher available.
Evidence:
The provider is unable to locate the fire extinguisher for the home during the inspection.

Plan of Correction: A new one has been ordered, and I will find the one that is in the house.
