Violations:
Standard #: 22VAC40-111-780-B
Description: Based on review of five children's records, the provider did not ensure one record was reviewed with the parent at least on an annual basis to ensure the emergency contact information is correct.

Evidence:
The record for Child #1 (start date 1/29/16) was dated as being last reviewed on 1/27/16.

Plan of Correction: I will have the family review the information and make any necessary changes today.

Standard #: 22VAC40-111-800-B
Description: Based on documentation review, the provider did not ensure the emergency plan was at least reviewed annually and update the plan as needed.

Evidence:
The emergency preparedness plan was dated September 2018. The provider stated she locked for the updated template but could not locate it on the Department of Social Services website.

Plan of Correction: An updated emergency preparedness plan was printed during the inspection and will be completed.

Standard #: 22VAC40-111-90-A
Description: Based on review of five children's records and interview, the provider did not obtain documentation that one child had been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations prior to attending the family day home.

The record for Child #5 (start date 1/13/2020) did not contain documentation of immunizations. The provider stated the parent has an appointment for the child on 2/03/2020.
Evidence:

Plan of Correction: I will make sure to get immunizations prior to children attending the family day home.
