Comments:

An unannounced monitoring inspection was conducted today approximately 11:05 am- 1:15 pm. There were 10 children totaling 27 points, directly supervised by 3 direct care staff. The physical plant, 2 staff records, 3 household record, 3 children?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. Medications were not on-site today. The playground and surfacing were inspected. The children were observed in free play in center activities and napping. Hand washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance. Description:	Based on record review, 1 of 2 staffing records does not have documentation of a current report of Tuberculosis screening every two years from the date of the first screening. Evidence: 1. Staff #2 last documented screening was dated 1/08/2017. 2. Household Members #1 last documented screening was dated 1/18/2017. 3. Household Member #2 last documented screening was dated 1/29/2017. 4. Household Member #3 last documented screening was dated 7/28/2016.
Plan of Correction:	Staff has a current TB and will send copy to licensing. I will get TB test for household members

Description:	Based on staff record review, one staff member did not obtain 16 hours of annual training. Evidence: 1. Staff #1 only completed 10 hours of annual training. 2. Staff #2 have not obtained 16 hours of annual training.
