Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on January 23, 2020 from 11:00 a.m. through 1:30 p.m. There were ten children in the care of the provider and one assistance for a total of twenty-six points. Five children's records were reviewed. The record for the provider, one household member and two assistants were also reviewed. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice.

Areas observed were lunch, hand washing/bathroom and diapering procedures, free choice, music and nap. Emergency supplies were checked as well as the water and refrigerator temperatures. Interactions between the children and providers were observed as appropriate and the children seemed comfortable with their daily routine and schedule.

Description: Based on record review, the provider failed to have documentation of an updated tuberculosis screening.

Evidence:
1. The records for the provider, one household member and two assistants were reviewed.
2. The provider did not have documentation of obtaining an updated tuberculosis screening.
3. The provider confirmed she had not obtained an updated tuberculosis screening within two years of the previous one.

Plan of Correction: I will obtain an updated tuberculosis screening as soon as possible. Documentation will be sent to the licensing inspector.
