Comments:
An unannounced monitoring inspection was conducted today from 10:30am-11:45am. There were 5 children (12 points) directly supervised by 1 staff. The home, 1 staff record, 2 children?s records, 4 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. The kitchen table was covered with paintings completed by the children early in the morning. At the start of inspection, children were playing outside on the porch with the provider. Children came inside with the provider and were playing with dolls, cars and other imaginative play toys. The provider was sitting on the ground and playing with the children.
The children played both independently and with the provider. There was a supply of books, toys, and materials for the children. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider did not maintain all information in children's records.

Evidence: Child 1 did not have written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds

Plan of Correction: I will have the parents sign in the emergency form tonight at pick up.
