Technical Assistance:
Technical Assistance was offered regarding: Hand washing and cribs.

Comments:
An unannounced monitoring inspection was conducted from 10:55 am through 12:50 pm. There were 11 children (34 points) with 3 caregivers present. A total of 5 children's files, 2 caregiver's and 1 household member's record was reviewed. The children were observed eating lunch that was brought from home. Children were also observed getting diapers changed, getting bottle fed and taking naps. There were no medications on site. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on record review, the family day home did not practice evacuation drills monthly.
Evidence: A review of the emergency drill record showed that evacuation drills were not completed for November and December 2019.

Plan of Correction: Drills will be completed and documented.

Standard #: 22VAC40-111-960-E
Description: Based on observation, infant bottles were not labeled.
Evidence: Infants milk bottles for three children in care were not dated.

Plan of Correction: The bottles will be checked for name and date.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical exam for children in care within 30 days of employment.
Evidence:
1. Child#1 's first day of attendance was 8/19/19.
2. Documentation of a physical exam was not obtained within 30 days of attendance.

Plan of Correction: I will request the physical exam from the parents.

Standard #: 22VAC40-111-110-B
Description: Based on record reviews, the immunizations records for children were not signed by a physician. '
Evidence: Immunizations records for Child # 1 and Child # 2 were not signed by the physician.

Plan of Correction: I will check for the immunization records to be signed and also remind the parents.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the family day home provider did not obtain subsequent Tuberculosis (TB) screening for an adult household member every two years.
Evidence:
1. Household Member # 1's most recent TB screening expired on 10/30/19.
2. There was no documentation to show that a subsequent TB screening was obtained.

Plan of Correction: The screening will be obtained and the documentation will be maintained in the record. I have to look for it.

Standard #: 63.2-1720.1-B-2
Description: Based on record review and staff interview, the family day home provider did not obtain fingerprint results for an employee before the first day of employment.
Evidence:
1. Caregiver # 1's record indicated that her first day of employment was 11/4/19.
2. The provider and Caregiver # 1 stated that Caregiver # 1 trained at the family day home for approximately one week with a former caregiver. Fingerprints were not obtained for Caregiver # 1 before this training period.

Plan of Correction: Fingerprints will be obtained before any new caregivers come into contact with the children.
