Technical Assistance:
Sixteen (16) hours of training hours are required annually.

Comments:
An unannounced renewal inspection was conducted today between 9:30 am and 12:45 pm. There were 10 children (34 points) with 3 caregivers present. The entire physical plant, children's records, household member's records, caregiver's records, cribs, emergency plans, menu's, attendance records, evacuation logs, emergency supplies, topical ointments and pet records were inspected. There were no medications on site. The children were observed hand washing, during diaper changing, singing and playing with floor toys. They were also observed eating lunch. Lunch consisted of ground turkey, rice, corn, peas, fruit and milk. Areas of non compliance were cited on Violation Notice. Standard #: 22VAC40-111-960-E
Description: Based on observation, the family day home failed to ensure that bottles in the refrigerator were labeled with the child's full name and the date, if more than one infant is in care.

Evidence:
1. Child #6's bottle was not dated.
2. One of chile #7's bottles was not dated and one bottle did not have her name or the date.

Plan of Correction: I will remind parents to label bottles and I will remind staff to label bottles if parents forget.

Standard #: 22VAC40-111-100-B
Description: Based on a review of children's records, the family day home provider failed to obtain a physical examination for each child enrolled prior to attendance or by the 30th day of attendance.

Evidence:.
1. Child #5's record does not have documentation of a physical. The child's first day of attendance was 03/02/2017.

Plan of Correction: I will ask the parent if they have a physical for the child.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the family day home provider failed to maintain required up to date records for children enrolled in the program.

Evidence:
1. Child #1's record did not have documentation of medical insurance.
2. Child #2's record did not have documentation of a physician's name address or phone number, names of persons other than the custodial parents who are authorized to pick up the child or a medical policy number.
3. Child #3's record did not have documentation of names of persons other than the custodial parents who are authorized to pick up the child or complete proof of identification.
4.Child #4's record does not have documentation of a medical policy number.

Plan of Correction: I will get the information from the parents and put in the fiels.

Standard #: 22VAC40-111-210-A
Description: Based on a review of annual training hours, the family day home provider failed to obtain and failed to ensure that caregivers obtain a minimum of 16 training hours annually.

Evidence:
Caregiver #1, #2 and #3 do not have documentation of the completion of 16 training hours for 2018-2019 year.

Plan of Correction: We will complete training hours for 2019-2020 year.

Standard #: 22VAC40-665-240-C-2
Description: Based on observation, the family day home did not ensure that electrical outlets shall have protective covers that are of a
size that cannot be swallowed by children.

Evidence:
In the first nap room, one of the electrical outlets did not have a protective cove

Plan of Correction: I will replace outlet cover today.
