Technical Assistance:
Licensing inspector discussed provisional license with the provider.

Comments:
An unannounced renewal inspection was conducted today from approximately 10:15am-11:40am. There were 3 children (9 points) directly supervised by 1 caregiver. A sample of 3 children?s records, 2 caregiver's records and 1 household member's record were reviewed. The children were observed napping and watching television. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. If you have any questions, contact Kimberly.Sawyer@dss.virginia.gov.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records were missing required information.

Evidence:
3 of 3 children's records reviewed were missing required information:
1. Child A was missing the first day of attendance, father's address, father's employer, father's employer phone number and address, insurance policy number, proof of identification, and field trip authorization.
2. Child B was missing the father's employer, father's employer phone number and address, and first day of attendance.
3. Child C was missing the address of one emergency contact, and field trip authorization.

Plan of Correction: Each parents have been notified to provide the missing information needed to complete the application.

Description: Based on record review, the provider did not obtain immunization records for each child in care.

Evidence:
Children A and B did not have immunization records on file.

Plan of Correction: Parents have been asked to proved immunization records for each child.

Standard #: 22VAC40-111-320-B
Description: Based on observation, protective covers were not used on all unused electrical outlets.

Evidence:
Two unused outlets located on a power strip in the main child care area were not covered with protective covers.

Plan of Correction: Protective covers will be placed on unused electrical outlets.

Evidence:
The central registry search of Household Member A was last completed 05/01/2014.
