Technical Assistance:
Reviewed background check forms required for household members.

Comments:
An unannounced renewal inspection was conducted today between the hours of 10:40 am through 1:30 pm with the provider on site. There were 2 children (6 points) with the provider. A sample of 2 children's files and 3 caregiver/household member's files were reviewed. The children were observed playing with toys, handwashing, eating lunch, bottle feeding, and during naptime. The physical space, evacuation drills, attendance records, and the Emergency Preparedness Plan were also reviewed. There were no medications on site. Standard #: 22VAC40-111-60-B
Description: Based on record review, one child's record did not contain all required information.

Evidence:
The record for Child #1 did not contain the name and policy number of the child's medical insurance, or names of persons other than the custodial parents who are authorized to pick up the child.

Plan of Correction: I will ask for the information today.

Standard #: 22VAC40-111-80-C
Description: Based on record review, the provider did not document in the child's record the method of verification of the child's age and identity.

Evidence:
Child #1 (start date 12/19/19) and Child #2 (start date 10/15/19) did not have documentation of who verified proof of identity or the date the proof of identity was verified.

Plan of Correction: Corrected during inspection.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for one adult household member a current Report of Tuberculosis Screening form every two years.

Evidence:
The last tuberculosis screening documented in the record for Household Member #1 was dated 11/29/2017.

Plan of Correction: A TB screen was completed. I will ask them for the documentation.

Standard #: 22VAC40-111-450-A
Description: Based on observation, doors with clear glass panels that reach within 18 inches of the floor were not clearly marked with decorative objects.

Evidence:
The sliding glass door exiting to the outdoor play area was not clearly marked with decorative objects.

Plan of Correction: I will put tape on the doors.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review, a central registry check was not obtained within 30 days of a person residing in the home becoming 18 years old.

Evidence: Household member #1 is over the age of 18. There is no documentation of a completed central registry check for household member #1. Provider stated the paperwork was returned due to error. Paperwork completed by provider was on the criminal history record check form.

Plan of Correction: Provider will provide documentation that the request was sent within 10 business days.
