Comments:
An unannounced monitoring inspection was conducted today between the hours of 11 am through 2 pm with the provider on site. There were 11 children (24 points) with the provider and one assistant caregiver. A sample of 4 children's files and 4 caregiver/household member's files were reviewed. The children were observed washing hands, eating lunch, reading books, playing with toys, learning about arctic animals, and preparing for nap time. The children were served lasagna with meat and strawberries for lunch. The physical space, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 4 children's records did not have all required information.

Evidence:
The record for Child #2 did not contain documentation of address and telephone number for the mother's place of employment.

Plan of Correction: I will ask the parent for the information today.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not obtain documentation that one child has been adequately immunized prior to their first day attending the family day home.

Evidence:
Immunizations obtained for Child #1 (start date 1/23/19) were dated 1/28/19.

Plan of Correction: Immunizations were obtained for Child #1 and placed in the file.

Standard #: 22VAC40-111-90-C
Description: Based on record review the provider did not obtain documentation of immunizations once every six months for one child under the age of two years.

Evidence:
The last documentation of immunizations in the record for Child #1 (age 16months) was dated 1/28/19.

Plan of Correction: I will ask the parents for updated immunization records.

Standard #: 22VAC40-111-180-A
Description: Based on record review, a current Report of Tuberculosis Screening form was not obtained every two years for one household member and one caregiver.

Evidence:
1. The most recent documentation of a tuberculosis screening for Caregiver #1 was dated 11/7/17.
2, Documentation of the most recent tuberculosis screening for Household member #1 was not available for review during inspection.

Plan of Correction: I will have them complete a tb test.
