Standard #: 22VAC40-111-880
Description: Based on observation, the provider did not ensure to follow the most recent, age appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
The inspector observed two after school children eat snack upon their arrival. Snack consisted of crackers and water. Snacks served by the provider must consist of two meal components, the provider only served one meal component to the children.

Plan of Correction: Typically do but will continue to serve 2 components. Meal was not claimed with USDA

Standard #: 22VAC40-111-40-A
Description: Based on observation, the Provider has failed to ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home.
The inspector observed five children in care upon arrival to the home on 1/14/2020. During the inspection two school age children arrived in the home, which increased the capacity. The Provider's licensed capacity is five. One of the two school age children is the provider's relative and household member, which does not count in the provider's licensed capacity. The provider had seven children in care and was over their licensed capacity by one child for approximately 30 - 45 minutes, until a parent picked up two children.

Plan of Correction: The parent currently picks up the school age child upon arrival from the school and that child will not attend in the summertime.

Standard #: 22VAC40-111-210-A
Description: Based on record review, the center did not ensure caregivers obtain a minimum of 16 clock hours of training annually in areas relevant to their job responsibilities.
The record of caregiver #2 did not have documentation of annual training.

Plan of Correction: Will obtain training and document forward.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure child resistant protective covers larger than 1-1/4 inches in diameter were installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
The inspector observed 3 uncovered outlets in the home where children are in care.

Plan of Correction: All covers were replaced while inspector was on site and have since been replaced with self closing covers.

Standard #: 22VAC40-111-530-F
Description: Based on observation, the provider did not ensure that no soft bedding of any kind was used under or around infants including, but not limited to, pillows, quilts, comforters, sheepskins, or stuffed toys.
The inspector observed child #1 (Age: 5 months) sleeping in a crib that contained one soft blanket, four stuffed toys, a blanket hanging over the front of the crib and a loose sheet around the mattress, which presented a suffocation hazard.

Plan of Correction: Removed stuffed animals from crib and room children are in. Have obtained tighter fitting sheets. Child (5) is in the same room as the provider and checked on frequently, as are other sleeping children by monitors.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not ensure children's hands were washed with liquid soap and warm running water before eating and after toileting or diapering.
The inspector observed the following:
1. Two school age children did not wash their hands after entering the home from the bus and before eating snack.
2. The provider diapered two children and did not wash the children's hands after diapering.

Plan of Correction: Will remind school children to wash their hands, and infants hands.

Standard #: 22VAC40-111-690-A
Description: Based on observation, the provider did not ensure children were not left unattended on a changing table during diapering.
The inspector observed the provider wash their hands in a nearby bathroom while leaving a child on a changing table. This occurred on two separate occasions. Each time the provider returned to the changing table within about 10 seconds. Children cannot be left unattended on a changing table.

Plan of Correction: Child was at arms reach for 5-10 seconds with 6" rails around. Will correct
