Technical Assistance:
Discussed with the provider what is required of a staff record if she needs an assistant and what is required of a household member's record based on their age.

Comments:
An unannounced renewal inspection was conducted today from 8:45am to 11:30am with the provider. There were four children in care, ranging from two months to four years of age, for a total of 11 points. Older children were observed having free play, watching a movie, washing hands, and preparing for lunch; the infant was observed waking from nap and being fed a bottle. Five children?s records, two staff records, and three household members? records were reviewed. The provider has current certification in CPR and First Aid. The provider does not administer medication and she is aware that she must obtain Medication Administration Training (MAT) in order to do so. Breakfast today was waffles, apples, and milk. The first aid kit, flashlight, and weather band radio were observed and were in compliance. The emergency drill log, attendance, and menu were also reviewed and were in compliance. The most recent evacuation drill was documented 1/13/20 and the most recent shelter-in-place was documented 1/14/20. Standard #: 22VAC40-111-60-B
Description: Based on review of five children's records, the provider did not obtain the full address and telephone number for two designated persons to contact in case of an emergency if the parent cannot be reached for each child. Evidence: 1. The record for Child A did not contain the telephone numbers of two emergency contacts. 2. The record for Child B did not contain full addresses for two emergency contacts. The record for Child C did not contain the full address for one emergency contact.

Plan of Correction: I will get that information from the parents today.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on review of three household member records and interview with the provider, the provider did not request a child protective services registry check within 30 days of a person in the home becoming 14 years old. Evidence: The record for Household Member A, with a birth day of 9/26/05, did not contain documentation of a central registry check. The provider stated that she had not performed the registry check yet.

Plan of Correction: I will complete the request today and put it in the mail.
