Comments:
An unannounced monitoring inspection was conducted today from 8:45am to 11:00am with the provider and one assistant. At the beginning of the inspection there were six children present, ranging from 18 months to four years of age, for a total of 16 points, with the provider. At 9:10 the assistant arrived, followed by three more children, for a total of nine children in care, ranging from 18 months to four years of age, and a total of 20 points. Children were observed having free play, being changed, resting, and doing worksheets about counting and colors. Six children?s records, the providers record, one staff record, one household members? record, and one pet record were reviewed. The provider and assistant have current certification in CPR and First Aid. There are currently no medications for children in the family day home, but the provider has current certification in Medication Administration Training (MAT). Meals are prepared by the provider and the posted menu for the week was observed and in compliance. The emergency drill log and attendance were also reviewed and were in compliance. The most recent evacuation drill was documented 12/27/19 and the most recent shelter-in-place drill was documented 7/30/19. Description: Based on review of six childrens records, the provider did not obtain documentation of a physical examination by a physician within 30 days after the first day of attendance for one child. Evidence: The record for Child A, with a start date of 9/30/19, did not contain documentation of a physical.

Plan of Correction: I will ask the parents for this again today.

Standard #: 22VAC40-111-90-A
Description: Based on review of six childrens records, the provider did not obtain documentation that a child had been adequately immunized before attending. Evidence: The record for Child A, with a start date of 9/30/19, did not contain documentation of immunizations.

Plan of Correction: I will ask the parents for this again today.

Standard #: 22VAC40-111-240-A
Description: Based on observation, not all areas or furnishings of the family day home were maintained in a safe condition. Evidence: The Licensing Inspector observed a small blue child's seat that had a large crack on the seat, posing a pinching hazard.

Plan of Correction: The provider removed the seat during the inspection and said it would be thrown away.
