Technical Assistance:
Technical assistance today included required paperwork, training opportunities, medication standards.

Comments:
An unannounced renewal inspection took place today from 10:30 am - 1 pm. There were 8 children at the beginning of the inspection for 23 points with the provider and assistant. I observed children playing with manipulative toys, listening to stories, eating lunch (with children complimenting the cook!) and preparing for nap. Standard #: 22VAC40-111-700-A
Description: Based on a review of medications today, one prescription medication did not have the prescription label as required

Plan of Correction: I will ask the parent to bring it in.

Standard #: 22VAC40-111-710
Description: Based on observation during today's inspection, not all conditions for administering medication were met.
Evidence:
There was no written authorization for the medication for Child # 6 from the parent.
The medication did not have a prescription label as required.

Plan of Correction: I printed the authorization and will give it to the parents today to complete and sign, and to get the doctor's signature for a long term medication.

Description: Based on observation during today's inspection, a child's medication was not kept in a locked box as required.
Evidence:
An emergency allergy medication was stored on a high shelf, but not in a locked place.

Plan of Correction: I will get a lock box for the medication.

Standard #: 22VAC40-111-920
Description: Based on observation during today's inspection, there was no menu available for review.

Plan of Correction: We will post the menu.

Standard #: 22VAC40-111-100-A
Description: Based on a review of records today, none of the children present today had documentation of physical exams as required.

Plan of Correction: I will print off the physical form and give it to parents as soon as possible.

Description: Based on a review of staff records, Staff # 2 did not have all required records.
Evidence:
There was no written documentation of two references for Staff # 2, who began 7/19.

Plan of Correction: I will make the calls and write down the information today.

Standard #: 22VAC40-111-60-B
Description: Based on a review of children's records today, not all required documentation was available.
Evidence:
Child # 4, 5, 6, 7 and 8 did not have the policy numbers for their health insurance as required. Child # 6 had only one emergency contact documented; 1, 3, and 4 did not have addresses of all emergency contacts. There were no signed permissions to go for nature walks/off premises for the children, and not all children's medication policies were consistent in terms of what the provider will administer.

Plan of Correction: We will get the information from the parents.

Description: Based on a review of staff records today, there was no documentation of an orientation for the assistant staffmember.
Evidence:
There was no written documentation of a training orientation as required for Staff # 2, who began 7/19.

Plan of Correction: She was trained but we will write the information on the form.

Description: Based on a review of staff training and staff confirmation, there was no documentation of 16 hours of annual training as required.

Plan of Correction: I took MAT training but we will take additional classes on-line.

Standard #: 22VAC40-191-70-A
Description: Based on a review of records, there was no Search of the Central Registry results for a household member as required.
Evidence:
Household Member # 3 turned 14 years old in July, but there was documentation of a Search of the Central Registry.

Plan of Correction: We will get that out as soon as possible.
