Comments:
The licensing inspector completed an unannounced monitoring inspection on 02/3/2020 from 10:10am to 12:30pm. The were no children in care upon the inspector?s arrival. An interview was held with the caregiver throughout the inspection.

The provider does not administer medication.
The provider does not provide transportation.
The provider does not have a pet.

Five children?s records, and two caregiver records were reviewed during the inspection.

Last shelter-in-place drill: 06/14/2019

The violations from the previous inspections were checked for corrections. There were no repeat violations found during today?s inspection.

Violations:
Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider did not ensure that one of five children's records contained documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations before the child may attend the center.
Evidence:
The record of child #5 (start date: 4/23/19) had documentation of immunizations. The immunization documentation was not signed or stamped and dated by a licensed physician, the physician's designee, or an official of a local health department.

Plan of Correction: I requested a new immunization record from the parent.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider did not ensure areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence:
1. The stairs in the home leading to the bathroom were missing 9 of the 16 support rails presenting fall and entanglement hazard.
2. There was a rusted pole with protruding sharp / flaking metal and paint in the providers backyard near the area where children play.

Plan of Correction: 1. I assist children when they go up and down the stairs. I will get the stair rails fixed in two weeks.
2. I will fix the rusted pole no less than two weeks from now.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider did not ensure child resistant protective covers larger than 1-1/4 inches in diameter were installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence:
There were 12 uncovered outlets in the family day home within the reach of children.

Plan of Correction: Corrected during the inspection.
