Comments:

An unannounced Renewal inspection was conducted this date. When the Inspector arrived at 11am, the two year old program was dismissing for the morning program. The afternoon program which consisted of 3 and 4 years olds arrived shortly after the morning program dismissed. A total of 9 children with Provider and 1 assistant was present for the afternoon program. When the children arrived, they gathered in circle time and the Provider read a book about snow. They broke out into three small groups, and transitioned to the art project that coincided with the book. The children made snow piles with shaving cream and glue. They discussed and then practiced snow angels. The programming area was decorated with Valentine's decorations made by the children. A walk thru of the upper level of the home was conducted, observation of the main programming area and bathroom downstairs was made. A total of 2 medications on site, both reviewed. A total of 5 children's records, 3 household member files and all staff (2) files were reviewed. The afternoon program discussed said the Pledge of Allegience, days of the week, the weather and counted and named months in Spanish. All staff observed encouraging children to participate in planned activities and engaged the children in dialogue when reading the book for the day. This inspection ended at approximately 2pm. Description:	Based on staff record review, documentation of annual training was not maintained. Evidence: Staff #1 and #2 obtained 16 hours of annual training. The certificates of the completed training was not maintained. The training was completed online through individual accounts, however written documentation was not maintained for the training.
Plan of Correction:	I will print the certificates of completion out and maintain the certificates in both files.
