An unannounced monitoring inspection was conducted today between the hours of 9:25 am through 10:30. There were a total of 8 children (26 points) observed with 3 caregivers. A total of 3 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: nap time, blocks, and trucks. Standard #: 22VAC40-111-60-B Description: Based on a review of children records, a child's record does not contain of the required information.
Evidence: Child #1, date of first attendance, 12/30/19 does not have the required authorization for emergency care, proof of identity and the signed page of the parent information form.Plan of Correction: I will get this information from the parent.
Standard #: 22VAC40-111-70-A Description: Based on a review of children records, there is not documentation before the child's first day of attendance, whether or not there is liability insurance.
Evidence: Child #1 date of first attendance 12/30/19 does not have documentation of whether is liability insurance.Plan of Correction: I will ask the parent to sign the form.
