Comments:
An unannounced monitoring inspection was conducted from 1:05 pm to 2:00 pm. At the time of inspector's arrival there were 9 children, ages 12 months-3yrs, for a total of 22 points, in care with the substitute provider. The records sample size consisted of 5 children's records and 2 provider / assistant/substitute records. Children were observed engaging in nap, free time, and interacting with the provider. The provider stated that no medications are being administered and there have been no injuries. Areas of noncompliance are detailed in the violation notice and were discussed with the provider during the exit meeting at the conclusion of this inspection.

Violations:
Standard #: 22VAC40-111-830-A
Description: Based on observation and interview it was determined the provider did not ensure that the emergency evacuation procedures were practiced monthly.
Evidence:
1. There was no evacuation drill logged for January 2020.
2. Provider confirmed that there was no emergency evacuation practiced in January 2020.

Plan of Correction: Provider will practice evacuation procedures monthly.

Standard #: 22VAC40-111-100-A
Description: Based record review and interview it was determined the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. The record for child #4(enrolled 12/5/19) did not contain documentation of a physical exam.
2. The provider confirmed the record for child #4 did not contain physical exam documentation.

Plan of Correction: Provider will request this documentation from parent at pick up.

Standard #: 22VAC40-111-90-A
Description: Based on record review it was determined the provider did not obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
1. The record for child #4(enrolled 12/5/19) did not contain documentation of immunizations.
2. The provider confirmed the record for child #4 did not contain immunization documentation.

Plan of Correction: Provider will request this documentation from parent at pick up.

Standard #: 22VAC40-111-240-A
Description: Based on observation it was determined the provider did not ensure that all areas of the home were maintained in a clean, safe, and operable condition.
Evidence:
1. The 4' bookcase in the child care area on the home was unstable and rocked when pulled on.
2. The provider confirmed that the bookcase was not anchored.

Plan of Correction: Provider will anchor the bookcase.

Standard #: 22VAC40-111-350-E
Description: Based on measurement it was determined the provider did not maintain hot water taps available to children within a range of 105?F to 120?F.
Evidence:
The hot water temperature was measured at 124.1?F.

Plan of Correction: Provider will lower the temperature on the hot water heater so that the temperature of the water is below 120 degrees Fahrenheit.

Standard #: 22VAC40-111-570-A
Description: Based on observation and interview it was determined the provider did not ensure that a caregiver does not exceed 16 points.
Evidence:
1. At time of the inspectors arrival there were 9 children in care totaling 22 points in care with the assistant provider.
2. The assistant provider confirmed the ages of the children in care and that the provider was not at the home.

Plan of Correction: Provider will hire another assistant/substitute provider and ensure that there is one caregiver for every 16 points present at all times.
