Technical Assistance:
The provider shall forward documentation of each caregiver's 16 hours of required training courses to the Inspector. This documentation must be received prior to the license expiration date of 3/5/20.

Comments:
An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 5 children, totaling 10 points. Children's records, caregiver/household records, background checks, caregiver training, emergency preparedness items, administrative records, child care programming and supplies, caregiver:child interactions, and children's medications were inspected today.

The children were observed completing academic work, playing with toys, singing songs, engaging in creative play, and preparing for lunch from home.

Violations:
Standard #: 22VAC40-111-700-A
Description: Based on a review of children's medications, it was determined that the provider has not obtained all required information on one child's medication authorization form.
Evidence:
One child's medication authorization form does not include a documented start and stop date for which the medication authorization is effective.

Plan of Correction: I will ask the parent to complete the missing information.

Standard #: 22VAC40-111-730-D
Description: Based on observations, it was determined that one child's medication was not stored in a manner that keeps it locked to prevent access by the children in care.
Evidence:
Upon arrival, the Inspector observed one child's prescription medication on top of the cubby shelf where the children store their food and belongings. This medication was not locked.

Plan of Correction: I will put the medication in the locking area of the kitchen.

Standard #: 22VAC40-111-800-B
Description: Based on a review of emergency preparedness items, it was determined that the provider has not reviewed the emergency plan at least annually.
Evidence:
The provider did not complete an annual review of the emergency plan. She most recently reviewed the plan on 8/31/18.

Plan of Correction: I will review the plan with my assistants this weekend.

Standard #: 22VAC40-111-120-B
Description: Based on a review of caregiver records, it was determined that the record for one assistant caregiver does not contain all required information.
Evidence:
The record for assistant caregiver B does not include documentation of two completed reference checks.

Plan of Correction: I forgot to do the reference checks. I will make sure to do them this weekend.

Standard #: 22VAC40-111-220-A
Description: Based on a review of caregiver training, it was determined that the provider has agreed to administer medication without holding the required training certification.
Evidence:
There was one child's medication on site during this inspection. The provider has not completed the required Medication Administration Training (MAT.)

Plan of Correction: I thought I did the right training, but it's the wrong course. I will register for the correct training ASAP.
