Comments:
An unannounced monitoring inspection was conducted today between 8:30 am and 11:00am. There were 6 children (14 points) present with 2 caregivers. The children were observed hand-washing, playing with play-dough, reading stories and working on an art project. The physical plant, attendance logs, accident reports, evacuation logs, menu's. cribs, children's records, caregiver records, household member records, pet records and emergency supplies were reviewed. Due to the weather the outside area was not inspected. There were no medications on site. Areas of non compliance were cited on the Violation Notice. Standard #: 22VAC40-111-760-A-1
Description: Based on observation, the provider did not ensure that the first aid kit contained required supplies.

Evidence:
The first aid kit was missing adhesive tape.

Plan of Correction: I will purchase adhesive tape.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the provider failed to maintain up to date records for each enrolled child.

Evidence:
1. Child #1' record does not have documentation of a medical insurance policy number.
2. Child #4's record does not have documentation of an address for the second emergency contact.

Plan of Correction: I will get information from parents.
